How a Hope ECM Care Manager Can Help
Enhanced Care Management (ECM) plays an important role in supporting individuals with disabilities and mental health disorders. ECM can significantly improve the quality of care and outcomes by providing comprehensive, coordinated, and individualized support. The services below are offered at Hope.
Hope's Behavioral Health Services Program
Hope Services’ Counseling Center provides mental health services for adults and children, ages five and up, with developmental disabilities and co-occurring mental health needs. A multi-disciplinary team provides all services.
We currently accept referrals from Santa Clara Family Health Plan (SCFHP) for ECM services.
